An application form and information sheet on Habia CPD endorsement is available to download from the Training and Skills section of the Habia website (Continuing Professional Development. The information we require to process an application includes:
- An agenda - (details of how long the course is, including time for breaks/lunch)
- Details on everything the course covers
- Everything that the learner would receive on the course (including, if an accreditation certificate is issued at the end)
- The course description
- A completed application form
- The fee.
